The following outline will assist the borrower entity, and its principals to know what essential information is needed in order to conduct an initial review of any loan request:

1. Project / Transaction Description - (one page or less).

2. Amount of Money Requested, currency, and how money will be used.

3. Collateral Description

• Summary - (one sentence).
Full Legal Description - (no matter how many pages).
Specifications, if applicable.
Name, Addresses, and Telephone Numbers of all relevant Recordation Jurisdictions, Venues, and Domains.
Original Purchase Date and Cost.
Estimated Value and Supporting Market Comparisons.

4. Disclosure of Borrower Entity
Articles and Certificates of Formation.
Resolution, (if applicable) specifying financing objectives.
Who has authority to make decisions.
Who will execute all documents on behalf of borrower entity for financing, etc.
Short resumes of all principal parties, including clean notarized photocopies of all principal party driver licenses and passports.

5. Borrower Entity Financials
Current Asset & Liability Statement - (one page).
Most recent Annual Profit & Loss Statement - (one page).
All Statements signed by Accountant, Attorney or Solicitor, or Banker including a short resume of signed party.

• Last Three (3) years Signed Tax Returns, all schedules, W-2's and 1099's.

6. Credit Enhancement
If Applicable, a copy of the text of the credit enhancement or loan guarantee instrument, name and address of the issuer, cost, and mechanics for issuance.
